The Matthew Project is looking to appoint a Living Well Worker as part of the development of Social Prescribing across Norfolk.

Living Well Workers are part of the development of Social Prescribing across Norfolk. Social Prescribing aims to address non-medical issues that may be causing or exacerbating health problems (e.g. debt, poor housing, mental ill health, social isolation, etc).
The Living Well Worker will advise and support individuals on a 1:1 basis, with the aim that they will experience improved health and wellbeing. Some of that support will be delivered directly by the Living Well Workers as well as through linking the client with a wide range of specialist services and support in the community. The service is intended to be time limited support, which encourages self-service and empowers clients to take charge of their next steps.
In the Breckland and King’s Lynn and West Norfolk District areas Social Prescribing is being delivered by a partnership of Age UK Norfolk, Community Action Norfolk, Family Action Swaffham, Future Projects, Home Group, The Matthew Project, West Norfolk Carers and West Norfolk Mind.
Each provider has a Living Well Workers to undertake the delivery of Social Prescribing.
